Extra virgin olive oil benefits: what the science actually says

'Is olive oil good for you?' is one of the most-searched food questions on Google. The short answer is: extra virgin olive oil is one of the best-studied foods in modern nutrition — but not all olive oil is equal. Here is what buyers, chefs and consumers should know, backed by peer-reviewed research.

The polyphenols do the heavy lifting

The EU health claim (Reg. 432/2012) authorises 'olive oil polyphenols contribute to the protection of blood lipids from oxidative stress' for oils containing at least 5 mg hydroxytyrosol and derivatives per 20 g. Only fresh, early-harvest EVOO — like our Picual — reliably clears that bar. Refined oils do not.

Oleocanthal: the peppery burn is anti-inflammatory

The throat-catching pepper of a good EVOO comes from oleocanthal, a natural compound that acts on the same COX-1/COX-2 pathway as ibuprofen (Beauchamp et al., Nature 2005). It is one reason Mediterranean diets show lower chronic inflammation markers.

Heart & cardiovascular health

The PREDIMED trial (NEJM 2013 & 2018) followed 7,447 adults and found a Mediterranean diet with ~50 ml/day of EVOO cut major cardiovascular events by ~30 %. This is the strongest dietary evidence base of any food outside of vegetables.

Brain & cognitive ageing

A 2024 Harvard cohort of 92,000 adults linked ≥7 g/day EVOO consumption to a 28 % lower risk of dementia-related mortality (JAMA Netw Open). Mechanism is likely polyphenol-driven reduction of neuroinflammation.

Metabolic & blood sugar

Meta-analyses (2019, Nutrients) show EVOO in place of butter or refined oils improves HDL, lowers LDL oxidation and modestly improves post-meal glucose response. Effect is dose-dependent and strongest with high-polyphenol oil.

How much per day

The clinical evidence sits at 20–50 ml (2–4 tbsp) per day, raw or lightly cooked. There is no benefit to megadosing; there is a real cost benefit to using fresh EVOO instead of refined seed oils for the same calories.

Drinking olive oil on an empty stomach — hype or help?

The 'shot of olive oil before bed / in the morning' trend has weak direct evidence but the underlying habit — substituting EVOO for refined oils — is well supported. A 15 ml morning tablespoon is safe for most adults; consult your doctor if you take blood thinners.
